Successful organizations are built by people; carefully designed systems support those people, they don't replace judgment. Every engagement starts with the human consequence of a decision — who is affected, what they will experience, what could go wrong — before any process, policy, or software mechanism is proposed to address it.

That means naming the actual trigger before naming a solution, being explicit about trade-offs instead of hiding them behind a scorecard, and keeping the final call with the people who are accountable for it.
